Responsibilities:
- Is responsible for the coordination of patient visits, maintenance and upkeep of scheduling records and log books.
- Staffing coordination duties include the accurate and timely communication of scheduling changes between office and field staff.
- Maintains a current client roster with necessary information.
- Updates logs and schedule sheets.
- Prepares weekly schedule for field staff and provides copies for distribution via email.
- Assists in coordinating services provided to patients.
- Controls and monitors schedule changes.
- Obtains weekly visit count and reports results to the Clinical Supervisor.
- Assists in relaying messages to field staff, office staff and community liaisons, gives and takes reports from the on-call coordinator.
- Takes on call on rotation and as needed
- Assist with confirming shifts and payroll as needed for private duty.
- Ensure geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care.
- Accurately process new patient referral information into the computer on a timely basis and create/maintain patient calendar.
- Acts as liaison among staff, patients, family, physicians, referral sources and payers
- Maintains an ongoing relationship with current, future and former clients, client families, company sales team, employees and outside agencies.
- Participates in company quality improvement activities.
- Understands and adheres to established policies and procedures of agency, as well as compliance to local state and federal laws.
- Answers telephone lines promptly, efficiently and channel them appropriately
- Performs other duties as assigned by the Supervisor, DCS or Administrator.
Qualifications:
- Home health experience required
- High school diploma or equivalent required.
- Demonstrates good communication and public relations skills.
- Passionate for delivering a world class customer experience.
- Internet service with minimum Broad band.
- Must be able to travel if indicated for training and/or Discovery Communities/Properties.
